What is a Personal Safety Action Plan ?

A Safety plan that:


Lists key activities in your life (home and work)
Identifies potential hazards associated with those activities
Prioritizes hazards based on risk (level of hazard x probability of occurrence) Develops an
action plan that will address the top ranked risks.

MY PERSONAL SAFETY ACTION PLAN, I will:

Sharing all Emergency telephone numbers like (Fire, hospital, etc.) with family to tackle

2 Holding Hand Rails while using staircases

3 Not using mobile while walking or driving

4 Parking my car in take off position


THE INCIDENT

o BHEL make BFP motor was planned for C schedule and it requires
shifting from site to Workshop during GT-4 HGPI in 2014. Motor Rating is 6.6KV,
400KW, Weight is 8 Tons.

o Crane was set to lift motor from pedestal and to put on the trawler.

o As soon as motor was lifted, one of the front leg of Crane went in soil
breaking the RCC flooring.

o Job was stopped. After putting proper supports (metal plates) below all
the Four legs of Crane, motor shifting job completed.
ROOT CAUSE

o Metal plates were not kept below all the Four expanded legs of Crane
imaging that the RCC flooring is tough and self sufficient to carry load.

o Job safety analysis (JSA) for carrying out the job was not done otherwise
it would have clearly indicated in the JSA for safety in crane operation.

o Poor judgment of rigger when load was lifted


Lesson Learnt

1. Crane should be positioned at proper place with four legs balanced


on hard surface with additional metal plates below all the expanded
legs.

2. JSA Should be conduct for any rigging activities / jobs.

3. Driver/Rigger to review load lifting procedure on case to case basis.
