BIPOLAR I DISORDER

At least 1 Manic episode


• Characterized as a period of time of at least 1 week
• Persistent elevated or irritable mood which present in most
days
• At least 3 of the symptoms below (4 if mood is irritable):
1. Inflated self-esteem or grandiosity
2. Decreased need for sleep (e.g. feels rested after 3 hours of sleep)
3. More talkative than usual or pressure to keep talking
4. Flight of ideas
5. Distractibility (i.e. attention easily drawn)
6. Increased in number of activities engaged (social, work, sexually
etc.)
7. Involve in activity that would self harm (unrestrained buying
spree, business foolish investments, sexual indiscretions)
BIPOLAR I DISORDER

• Marked impairment in social or occupational


functioning, necessity for hospitalization to
prevent harm to self or others or there are
psychotic features
• Manic episodes triggered by antidepressant
usage or ECT, the diagnosis of bipolar I
disorder could be made if symptoms persist
even upon discontinuation of existing
treatment
BIPOLAR II DISORDER

• Past or current hypomanic episodes


• At least 4 days of persistent elevated or irritable mood
present on most of the days
• At least 3 of these symptoms (4 if the mood is irritable)
1. Inflated self-esteem or grandiosity
2. Decreased need for sleep (e.g. feels rested after 3 hours of sleep)
3. More talkative than usual or pressure to keep talking
4. Flight of ideas
5. Distractibility (i.e. attention easily drawn)
6. Increased in number of activities engaged (social, work, sexually
etc.)

BIPOLAR II DISORDER

• NO marked impairment in social or


occupational functioning, necessity for
hospitalization to prevent harm to self or
others. If there are psychotic features, it is
manic already.
• NO previous manic episode
MIXED EPISODES AND RAPID CYCLING

• Mixed episode:
Fulfill both manic and major depressive symptoms for at
least 1 week

• Rapid cycling:
Consist of at least 4 episodes of mood disturbance (manic,
hypomanic, depressive episode) in 1 year

• Ultra rapid cycling:


4 or more episodes in 1 month
OTHER SPECIFIED BIPOLAR AND RELATED
DISORDER
• Hypomanic episodes of short duration (2-3 days) and
major depressive episode.

• Hypomanic episodes with lack of symptoms and major


depressive episode.

• Hypomanic episodes without major depressive


episode.

• Cyclothymia of less than 2 years duration.
