No. 8 s.2016 Guidelines in assessing and determining hazardous work in the employment of persons below 18 years old *15 and above- 8:00 a.m to 5:00 p.m *below 15- four hours with permission from the guardian or parents *shall not be a replacement or diminution of the benefits of the regular workers II. ACADEMIC CREDIT *meet with the WI coordinators for student eligibility *Prepare the following: -endorsement letter -application letter -standard resume -waiver form -DTR -PAF *prepare for an exam and interview *confirmation of work immersion placement II. ACADEMIC CREDIT Submit the following: -HTE Enrolment Form if any -WAIVER FORM -attend MANDATORY ORIENTATION meeting -approved TRAINING PLAN DURING the Training IMPORTANT things to remember at WORK! 1. Maintain DTR 2. Keep a daily training journal 3. Submit on time!!! 4. Let the on-site supervisor to fill out the PAF and discuss results 5. Submit DTR and PAF to the WIC with Work Integrated Paper -Draft –after 40 hours of training -Final paper- after 80 hours 4th quarter exam CONSULTATIONS 1st Consultation- after signing the student training contract 2nd Consultation- rendered 40 hours 3rd Consultation- submission of the work immersion Integrated paper AFTER the Training REMINDERS!!! 1.
