Problem Definition
• We want to create a comprehensive knowledge-base of the patients
information, so that the doctors can easily access a patient's health records,
their previous medical history, family medical history etc. based on which the
doctors can provide correct treatment, prevent any foreseeable diseases,
prescribe the right medicines and so on.
Methodology
How are you collecting patients data?
 Survey is conducted once in 15 days and the blood samples collected are sent
to labs.
 Patients visiting the PHC (Primary Health Care Centre)

How are you processing the data?


 Blood samples are tested in lab.
 Data is entered in the software and recorded.
How is a doctor accessing the data?
 Doctor can access the data using medical record number that is assigned to
the patient.
 He can also use Aadhar number or Ration card number to fetch his details.

How is he using the data?


 Analysis the data and provides medication accordingly.
 Detects diseases at early stage and cures it.

• Front End : HTML, JQUERY, BOOTSTRAP, JSON, CSS, JAVASCRIPT,
AJAX.
• Back End : PhpMyAdmin MySQL(version 5.6.36)
