• Concerns about the possible encouragement of antimicrobial
resistance have led to recommendations by some clinicians that
antimicrobial agents be withheld in large subgroups of children
with acute otitis media, unless symptoms persist or worsen.
• A potential strategy for reducing the risk of antimicrobial
resistance is to limit the duration of antimicrobial treatment.
Time : We conducted this trial from January 2012 through
September 2015.
Exclusion Criteria
• Children who had tympanic membrane perforation or another illness
• Allergic to amoxicillin
• Received more than one dose of an antimicrobial agent within the previous
96 hours
Daily dose :
10 days amoxicillin– 90 mg of amoxicillin
clavulanate, or and 6.4 mg of clavulanate
per kilogram of body weight
- 6-11months
-12-17 months We randomly assigned
children in blocks of four
- 18-23 months
5-days amoxicillin–
5 days amoxicillin– clavulanate
clavulanate followed by
5-day placebo
We assessed children on day 4, 5, or 6 by means of telephone conversations with
the parents.
We also asked parents to record their child’s AOM-SOS scores daily.
Assessments every 6 weeks through the end of the respiratory infection season
(defined as October 1 through May 31),
at interim visits for illness, and at an end of study visit.
The Primary Measure The Secondary Measure
• The percentage of children who had • Symptom burden over the period from
clinical failure after treatment. day 6 (when placebo use in the 5-day
group began) to day 14 of the index
episode
• Rate of recurrences of AOM.

acute otitis media
• Total days of antimicrobial treatment
during the respiratory infection season.
• Rates of nasopharyngeal colonization.

Design Study :
• All the analyses were performed with the use of twosided tests
