 History

• Pulsating headache in • Shortness of breath,

 History of present illness
Anamnesis done on July, 15th 2018 :
• Mr. A came with a shortness of breath since a week before admitting to the hospital, felt
heavier ±3 days before being hospitalized, shortness of breath accompanied by cough,
cough mixed with blood in the morning before hospitalized, coughing blood approximately
already as much as 30 tissue. Patients are consulted to the neurological department with
dizzy spinning complaints, dizziness is not affected by the change of position from sleep to
sitting or vice versa, dizziness felt at the times patient already treated in the hospital, other
than that the patient felt weak and difficult to walk and need guided when he's walking.
• And now patients complain of front headaches, pain felt throbbing, pain felt for
approximately 30 minutes and continuously, lost and arisen, the patient also felt weak and
the need to be guided when walking and additional complain Shortness of breath, body feel
limp, decrease appetite, coughing blood has started to decrease
