    5-May-18 10:15:00 North Load 6 (Medium ) 564902 - Shikhar Kumar while conducting the ventilation survey at 285mRL, 1 piece of pipe was lying
    Gupta in the drive in the water accumulated there. Suddenly the foot of the person
    accompanying was placed on the pipe and he slipped but somehow
    managed to fall.

    4-May-18 5:55:00 VRM & East Load 4 (Low ) 564892 - Sarth Manish While a loaded LPDT was passing through 100mRL north (near surface
    Bodawala ramp), while turning it got strike on side wall and thus cracked front glass of
    LPDT. Luckily no injury occurred to operator.

    2-May-18 19:45:00 VRM & East Load 8 (Medium ) 576699 - Rakesh Singhadia AT 100mRL E , 2 LPDT'S WERE GOING UPRAMP ONE LPDT WAS BREAKDOWN
    ON THEIR PATH SO THEY CAME OUT OF THE THEIR RESPECTIVE LPDT'S AND
    PARKED THIER LPDT'S ON THE RAMP. BUT WHEN THE LPDT( WHICH WAS
    BD) GIVEN OK. 1ST LPDT OPERATOR STARTED HIS VEHICLE AND TRY TO
    MOVE IT BUT SUDDENLY THE SECOND LPDT OPERATOR TRY TO CROSS THE
    LPDT FROM LEFT SIDE WHICH WAS SO CONFINED AND THERE WAS NOT ANY
    MAN HOLES TO SAVE IT BUT EVENTUALY HE NEARLY ESCAPED FROM ANY
    INJURY.
