1. Chronic Disease Management Chronic diseases are a significant cause of

problems such as diabetes, hypertension,
high blood cholesterol and stroke are
lifelong conditions. The programme helps
patients take charge of their health and
manage these conditions in order to lead a
fulfilling life.
2. Human Organ Transplant Act (HOTA) To help save more lives and meet the
increasing demand for organ
transplantation in Singapore, HOTA
allows for the kidneys, liver, heart and
corneas to be removed in the event of
death for the purpose of transplantation. It
also regulates living donor organ
transplantations.

3. Advance Medical Directive (AMD) To help Singaporeans manage aging
issues, the AMD allows any Singaporean
aged 21 and above to indicate their
decision to not receive medical treatment
should they become terminally ill or
unconscious. This is necessary as new
advances in medical knowledge and
technology can artificially prolong life in
the final stages of a terminal illness.
