DEFINISI

 Heart Failure (HF) is a complex clinical syndrome that results from any structural or functional impairment of
ventricular filling or ejection of blood (AHA 2013)
 HF is a clinical syndrome characterized by typical symptoms (e.g. breathlessness, ankle swelling and fatigue) that
may be accompanied by signs (e.g. elevated jugular venous pressure, pulmonary crackles and peripheral oedema)
caused by a structural and/or functional cardiac abnormality, resulting in a reduced cardiac output and/or elevated
intracardiac pressures at rest or during stress. (ESC 2016)

NYHA Functional CLASS AHA STAGE

I No limitation of physical activity. A No structural heart disease, risk factors for HF
Ordinary physical activity does not cause symptoms
of HF
II Comfortable at rest, slight limitation of physical B Structural heart disease, no HF symptoms
activity
III Comfortable at rest. Marked limitation, less than C Structural heart disease, with prior or current HF
ordinary activity causes discomfort symptoms
IV Discomfort at rest D Refractory HF requiring frequent interventions

DIAGNOSIS
BAGAIMANA DG CHEST X-RAY?

 A chest X-ray is of limited use in the diagnostic work-up of patients with suspected HF
 It is probably most useful in identifying an alternative diagnosis
 The chest X-ray may, however, show pulmonary venous congestion or oedema in a patient with HF,
and is more helpful in the acute setting than in the non-acute setting
 It is important to note that significant LV dysfunction may be present without cardiomegaly on the
chest X-ray
