    SUBJECT:Communication Cable Cut 10/12/2006 Hawiyah NGL Recovery Plant Project.


    Location : HGP/ Area- A /B/C, Inlet area and Area-D Date : 11February,2007.
    While excavating a trench using Backhoe#6/2.04/H-12, the operator cut a satellite office communication cable at PAD #10.

    Basic Causes:
    - The work was done without a JGC/CCC site excavation work permit.
    - Poor supervision
    Recommendation to prevent repetition of the situation:
    - No excavation work is to be done without a work permit
    - Follow the company’s loss prevention program full when excavation work is performed.
    - This safety alert will be discussed in toolbox meeting, construction meetings with all supervisors.
