Adoption of New Forms for Kindergarten, Senior High
School, Alternative Learning System, Health and
Nutrition and Permanent Records

• This policy institutes new forms to be used in SCHOOLS and


OTHER INSTITUTIONS delivering basic education, particularly
Kindergarten, SHS and ALS and standardizes the forms for the
learners’ Health and Nutrition, and permanent Records.

• The new STANDARDIZED FORMS shall be used in all Public


Schools and ALS Community Learning Centers starting SY
2017 – 2018. (Private Schools, state and local colleges/
universities are also enjoined to adopt said forms)
DepEd Order 58 s. 2017
• The use of these forms in all Public Schools is MANDATORY.
No other forms will be used as official documents in public
schools nationwide unless approved by the Office of the
Undersecretary for Planning and Field Operations.
• This policy shall lessen the time and effort of teachers and
school heads for administrative matters to allow them to
focus more on their core business, the delivery of the
curriculum.
• These new forms are available for download from the DepEd
Official Website (www.deped.gov.ph) under the RESOURCES
TAB and from the Learner Information System
(www.lis.deped.gov.ph) under SUPPORT PAGE.

SCHOOL FORM 9 – Learners’ Progress Report Card
- An individual, periodic report of a learner’s academic achievement per grade
level
- The Progress Report Card was renamed to Learners’ Progress Report Card or
SF9. THE FORMAT AND CONTENT OF THE EXISTING PROGRESS REPORT CARD
AS PRESCRIBED BY DO 8. S. 2015 WILL STILL BE ADOPTED IN ALL GRADE LEVELS
EXCEPT IN KINDER (KINDERGARTEN PROGRESS REPORT, DO 47 s. 2016).

SUMMARY OF FORMS
SCHOOL FORM 10 – Learners’ Permanent Academic Record
- An individual record of a learner’s academic achievement per level
- The Permanent Record commonly known as FORM 137 was renames to
Learners’ Permanent Academic Record or SF10.

GENERAL GUIDELINES:
a. To simplify the format, the SF10 will only bear the DEPED OFFICIAL LOGO
AND SEAL

b. Effective SY 2017 – 2018, the updated and standardized SF10 will be adopted
for GRADE 1 and GRADE 7.

c. For other grade levels (Grade 2-6 and Grade 8-10) the OLD EXISTING
FORMAT OF FORM 137 will be used until the learner complete the
ELEMENTARY OR JUNIOR HS LEVEL. THERE IS NO NEED TO RENAME TO SF 10

d. INCOMING GRADE 11 LEARNERS will continue using the existing format and
content as prescribed on DO 69 s. 2016 but will rename to SF10-SHS.

SF10-ES
- To be used by ELEMENTARY SCHOOLS
GUIDELINES:
a. G1 Teacher shall prepare one (1)
copy of SF10-ES and attach relevant
supporting documents (a copy of
Birth Certificate, Kinder Completion
Certificate/ECCD Checklist, Progress
Report or other equivalent
document)

b. If learner continues in the same


School, G1 adviser forwards said
documents to G2 adviser. G2
adviser shall make necessary
updating to the SF10-ES. Same
procedure will be followed until
learner finishes G6.

c. If the learner TRANFERS OUT, the
RECEIVING SCHOOL shall request the
transfer of ORIGINAL COPY of SF10-
ES and other Pertinent Documents.
The ORIGINATING SCHOOL, upon
receipt of written request or system
notification via the LIS (whichever
comes first) shall create one certified
true copy of the SF10-ES for their file
and send the ORIGINAL SF10-ES
along with PERTINENT DOCUMENTS
to the Receiving School.

The RECEIVING SCHOOL shall


UPDATE the notification in the LIS
once the documents have been
received and shall continue updating
the SF10-ES of the Learner.

d. The SCHOOL WHEREIN the
LEARNER completes his/her
ELEMENTARY EDUCATION shall
keep the ORIGINAL COPY OF SF10-
ES.
e. Upon graduation and transfer to
Secondary School, the Originating
Elementary School will prepare a
CERTIFIED TRUE COPY of Learner
SF10-ES along with other pertinent
documents and send it to the
Receiving Secondary School upon
receipt of written request or
system notification.
f. The G7 adviser will use the
Certified True Copy as reference in
the preparation of SF10-JHS

- Should the learner request for a
copy of SF10-ES for employment or
for any other reason EXCEPT
enrolment to JHS. The School will
prepare a certified true copy INK
signed by the School Head bearing
School Logo.

- Any alteration or erasure shall bear


the SCHOOL HEAD’S SIGNATURE IN
INK
- SF10-ES shall be prepared at end
of School Year or in the case of a
learner transferring out

- To be used by JUNIOR HIGH SCHOOLS
GUIDELINES:
a. G7 Teacher shall prepare one (1)
copy of SF10-JHS and attach the
certified true copy of SF10-ES and
other relevant supporting
documents (a copy of Birth
Certificate, Progress Report or
other equivalent document).
b. If in the case that the learner
does not have an available SF10-ES
due to acceleration. The CERTIFIED
TRUE COPY of ASSESSMENT
RESULT/CERTIFICATE shall be used
as reference and attachment to
SF10-JHS.

c. If learner continues in the same
School, G7 adviser forwards said
documents to G8 adviser. G2
adviser shall make necessary
updating to the SF10-JHS. Same
procedure will be followed until

IMPLEMENTATION GUIDELINES
1. NEW FORMS adopted in this policy will be available with pre-
populated data for downloading from the LIS WEBSITE.

2. NEW FORMS are available for download (including instructions


on its usage are available at www.deped.gov.ph (under
RESOURCES TAB) and www.lis.deped.gov.ph (under SUPPORT
TAB)

3. To ensure consistency and compliance with the standardized


format, downloading of electronic forms from unauthorized
website or social media is DISCOURAGED.
IMPLEMENTATION GUIDELINES
4. The SCHOOL HEAD is responsible for organizing
orientations/meetings/activities to ensure that teachers are
knowledgeable in the use of the NEW FORMS. The SGOD may
extend technical assistance to support the said activities.

5. The ALS FOCAL PERSON under the CID shall also conduct
orientation/meetings to inform ALS Teachers and Learning
Facilitators
MONITORING AND EVALUATION

The Schools Division Office through the SGOD, is


likewise mandated to monitor compliance regarding
this policy. Any policy-related issues or non-compliance
with these guidelines shall be reported to the Central
Office – through the Planning Service.
