3. Clasificación de pacientes con hepatitis B de
acuerdo a exámenes
Grupo A. Antecedentes de infección e inmunidad
natural a VHB: Anti-HBc (+), Anti-HBs (+), HBsAg (-)
Grupo B. Portador inactivo: HBsAg (+), HBeAg (-),
transaminasas normales, carga viral <104 copias/mL.
Grupo C: Inmunotolerante: HBsAg (+), HBeAg (+) ,
transaminasas normales, carga viral >104 copias/mL.
Grupo D: Hepatitis B crónica replicante: HBsAg (+),
HBeAg (+), Anti HBe (-), transaminasas aumentadas,
carga viral >104 copias/mL, biopsia hepática consistente
con hepatitis crónica.
4. Protocolo de seguimiento de portadores de
hepatitis B
Grupo A: no requiere seguimiento
Grupo B: Portador inactivo
Evaluación clínica cada 4-6 meses
Perfil de función hepática cada 4-6 meses
Serología VHB cada 4-6 meses:
Antígeno de superficie HBsAg
Antígeno de replicación HBeAg

Si se decide tratar: protocolo de tratamiento
La periodicidad podrá ser modificada de acuerdo a la evolución del
paciente y evaluación del equipo tratante
5. Protocolo de tratamiento propuesto
a. Pacientes con antecedentes de infección por VHB y portador asintomático.
No requieren tratamiento
b. Inmunotolerante
El tratamiento va a depender de la evaluación del equipo consultor
c. Hepatitis B crónica replicante:
La opción de iniciar tratamiento se individualizará según los resultados de la biopsia
hepática, magnitud de la fibrosis y grado de inflamación. Si se decide no tratar, se
seguirá según protocolo cada 4 a 6 meses y se re-biopsiará en 3 a 5 años.
En los niños en que se decida tratamiento, las opciones son el uso de interferón-α y
lamivudina.
I. PROTOCOLO LAMIVUDINA/INTERFERON
Lamivudina:
Uso a partir de los 2 años de edad
– Dosis: 3 mg/kg/día, una vez al día. dosis máxima 100 mg/día
– Indicación: primeras 8 semanas como monoterapia, seguido de
44 semanas asociado a interferón-α
d. Chequear que todo el personal de salud que atienda a estos niños cuente con la vacuna anti VHB al día
e . Contactos sexuales de adolescentes portadores de hepatitis B:
Contactos no vacunados, indicar una dosis de HBIG (0,06 mL/k IM), dentro de los primeros 14 días del contacto y
comenzar vacunación.
Las parejas sexuales estables deben recibir vacunación
