4. Check Accuracy of Document

If there are any inconsistencies, note down in SFCR1 Table 2
Reminder:
One of the common errors observed during
the checking of forms in the previous school
year is the wrong usage of the terms
“transferred out” as against “moved out” and
“transferred in” as against “moved in”. Please
ensure that the correct terms are used
especially in the LIS.
5. Check the Enrolment Count and
Learner Movement (SGOD)
Transferred out/in – If the learner transferred to/in another school within the
same school year
Example: Student A who enrolled as a grade 4 student of Tarragona CES (SY
2018-2019) transferred to Cabagayan ES last August 2018.
Moved out/in- If the learner transferred to/in another school in between two
school years
Example:
Student A who was once a grade 4 student in Tarragona CES (SY 2017-2018)
enrolled as a Grade 5 student in Cabagayan ES the following school year (SY
2018-2019)
6.1 Collect SF 4,5 & 6 (SGOD)
