1.1 Add the total enrolment of Kinder, Grade


1 and Grade 6 (ELEMENTARY)

percentage of classes to be checked. See table below:

1.2 After getting the total, identify the size of the school and the
number of classes to be checked. See table below:

1.3 Add the number of sections of the following grade
levels:
a.Kinder, G1 and G6 (ELEMENTARY PRIORITY LEVELS)
b.G2, G3, G4 and G5 (ELEMENTARY-OTHER GRADE LEVELS)
c.Grade 7 and 10 (JUNIOR HIGH PRIORITY GRADE LEVELS)
d.Grade 8 and 9 (JUNIOR HIGH OTHER GRADE LEVELS)
e.Grade 11 and 12 (SENIOR HIGH PRIORITY GRADE LEVELS)

4. Check Accuracy of Document
Entries
 If there are any inconsistencies, note down in SFCR1 Table 2
Reminder:
One of the common errors observed during
the checking of forms in the previous school
year is the wrong usage of the terms
“transferred out” as against “moved out” and
“transferred in” as against “moved in”. Please
ensure that the correct terms are used
especially in the LIS.
5. Check the Enrolment Count and
Learner Movement (SGOD)
 Transferred out/in – If the learner transferred to/in another school within the
same school year
Example: Student A who enrolled as a grade 4 student of Tarragona CES (SY
2018-2019) transferred to Cabagayan ES last August 2018.

 Moved out/in- If the learner transferred to/in another school in between two
school years
Example:
Student A who was once a grade 4 student in Tarragona CES (SY 2017-2018)
enrolled as a Grade 5 student in Cabagayan ES the following school year (SY
2018-2019)
6.1 Collect SF 4,5 & 6 (SGOD)

These shall be collected after


all grade levels have been
checked

missing data entries

General Reminders
TOTAL NUMBER OF CLASS DAYS FOR SY 2018-2019
2. TOTAL NUMBER OF SCHOOL DAYS
FOR SY 2018-2019
DepEd Order No. 25, s. 2018 otherwise known as
DepEd School Calendar for School Year 2018-
2019 shall end on April 5 (Friday) and shall
consist of 200 days inclusive of 5 days INSET and
3 days Parent-Teachers Conferences
conducted within the school year.
6. New and Old form 137 (SF10)

The new Form 137 (SF10) shall be used by


the present Grades 1-2 and Grades 7-8.
All other Grades shall use the old/existing
Form 137 until the learner completes
elementary/ junior high school level.
NSO/PSA Birth Certificate must be
attached to the Form 137.
WAYS FORWARD

For ease in identifying the sections to


be checked, the schools are advised
to prepare the following
Enrolment count of every grade level
and number of sections per grade
level
