Readiness Overview

Õ Vo be fully medically ready, also known as ´greenµ, an
individual must meet all six of the criteria.

1. M  
   (annual)- PHA fully
implemented CG-wide MAR09. (See ALCOASV
118/09).
As of Jan 2010, the 5- Year Physical Examination will
no longer be used as a proxy for the PHA (and will
no longer be reflected in CGBI). If CG members do
not complete their PHA during their birth month
period, their PHA metric will turn red in CGBI
(See ALCOSV 105/10)

2. Dental- Class 1 or 2

3. Immunization readiness now includes:


Vetanus, Influenza, MMR, Polio (oral or injectable ),
Hep A & Hep B vaccinations.
In order for a member to be 100% immunization
ready, they must have a current immunization for all
6 immunizations.

4. Medical Equipment: 2 pr glasses & if expeditionary,


1 pr mask inserts

5. Medical Readiness Labs: DNA, G6PD, Sickle cell,


baseline PPD, HIV q2yrs

6. No deployment limiting conditions per Chapter 3


of Med Manual.

in 12 months could cause a dental emergency.

charge); exam and DENCAS entries done on site.

Õ Use private dentist (have DD-2813 completed and


faxed to local CG Clinic or MLC (k)/ HSWL for data
entry, if remote)

PHA Appointment and Dental Appointment (if needed)
2. Complete the leet Health Risk Assessment (HRA)
online

HRA questions, print a copy for appt. and bring it with
you.
3. Vhe Kit will be sent to your home by the RHRP
contractor. Bring your PHA Kit to the appointment
4. Bring your medical devices (Hearing aids, glasses, dental
orthodontic equipment etc.) to the appointment.

5. SELRES Members are authorized and should request
one Readiness Management Period (RMP) in Direct
Access for the date of the PHA.
6. All cancellations must be done at least 72 hrs prior to
the scheduled appointment.
7. Make follow up appointments as needed with your
Primary Care Manager. It·s the member·s responsibility
to complete any follow up appointment.
8. Verify PHA updates in CGBI 2.0
