Land  Full description of the land;
Licensed  Citizenship and civil status of the applicant;
Surveyor Management
 Full names and addresses of all occupants of the
Bureau land and those of the adjoining owners;
 Assessed value of the land and buildings and
improvements thereon;
 Whether or not there are encumbrances of any kind;
 The manner of acquisition;
Approved by Regional  Whether or not the property is conjugal ,
Executive Director, paraphernal or exclusive property of the applicant;
 Original muniments of title and other related
Land Management
documents supporting applicant’s ownership;
Service of the DENR  If the land is bounded by a public or private way or
road, whether or not the applicant claim any and
what portion of the land within the limits of the way
or road, and whether the applicant desires to have
the line of the way or road determined.
Non-resident Applicant

Representative:
• SPA executed in the foreign country certified
by any of the following:
- secretary of embassy;
-consul general;
-vice consul;
-consular agent; or
- any officer in the foreign
service of the Philippines
stationed in the foreign
country.

Service of all papers and other legal processes


shall be made upon the representative or
attorney-in-fact with the same effect as if made
upon the applicant himself
