Department Visited: Slitting Section
• Slitting basically a shearing operation that cuts a large roll of material
into narrower rolls.
• Slitting Section contain total 13 Machines for slitting
• 3 Machines are Titan and one is Web Control
• 10 Machines are EUROMAC
• Purpose of all machine is to perform slitting but difference is in
machine features
• We Perform common cause failure analysis on EUROMAC Machine

Common Cause Failure Analysis ( CCA)
• Subset of Dependent Failures in which two or more component fault
states exist at the same time, or within a short time interval, as a
result of a shared cause.
• Common Cause Failure Analysis (CCFA) is an extension
• of FTA to identify "coupling factors"
• This analysis provides additional insight into system failures after
development of a detailed FTA when data on
• components
• physical layout
• operators and inspectors are available

Significance:
Defeat the redundancy employed to improve the reliability
of safety functions.
Operating experience has shown that CCF are major
contributors to plant risk.
CCF attributes:
A shared cause exists
The shared cause has two elements, a root cause and a
coupling factor:
Root cause: Why did the item fail? (i.e., linked to the item)
Coupling factor: Why were several items affected?
(i.e., linked to the relationships between several items)
