NARRATIVE REPORT

 The purpose of the narrative report is to supply


a description of your contributions and
experiences as a trainee. The narrative
should be integrative, demonstrating the
relation across various activities in which you
are a part of.
In writing the narrative….

You should take note of the following:


 Narratives are generally written in the first person

(i.e. using I). However, third person (he, she, or it)


can also be used.
 Narratives depend on specific, sensory details to
impart a point.
 Narratives, as stories, should include these sensory

agreement: a plot, including setting and characters;


a climax and an ending
The student-trainee shall prepare an OJT
Narrative Report after fulfilling the 600-hour
practicum.
The narrative report shall contain the
following:

 Acknowledgements
 Table of contents
 Recommendation/Endorsement Letter
 Certificate of Completion
 Letter of Acceptance
 MOA and/or waiver

 Experiences (weekly accomplishments)
 Lessons learned/Realizations
C. General Evaluation
 General impressions and Recommendations
(overall personal assessment of the OJT
experience)
APPENDICES
Scanned copy of the following:
A OJT Evaluation Form
B Daily Time Record
C OJT Time Table
D Registration Form and Official receipt
E Curriculum Vitae/Resume
F Student Health Record
G Certificate of Good Moral
H Application Letter
I Job Description (if any)

A. Text F. Pages
- Arial Narrow font, size 12 - should be written at the upper right hand corner of
B. Margin (inside pages) each page except when chapters/topics are written,
- Left margin = 1.5 inch in which case no page number is written but a
- Upper, lower and right margin = 1 inch number is assigned for it in the sequence.
- Pages for preliminary contents should be i, ii, iii, iv,
