STANDARD PRACTICE - CODE 4 REF. NO. 4.14

BRAKE TESTING SYSTEM


OBJECTIVE

To provide Technicians, Artisans, Operators and Supervisors with the procedure to be followed
when the effectiveness of the braking systems on the vehicles are tested.

4.14.1 BRAKE TEST FREQUENCY, SEQUENCE METHOD AND CONTROL

1. ADT’s and LHD’s braking systems will be tested by the operator prior to commencing work
in the presence of an appointed Technician or Artisan.

2. All vehicles braking systems will be checked by the Artisan or Technician on a designated
ramp after any routine service.

3. No vehicle which has had brakes repaired or replaced will be allowed to proceed to work
unless the brakes have been tested on a ramp and found to be effective by the operator and
the Artisan / Technician in charge of the repairs.

4. The Workshop / Contracts Manager and the Engineer will ensure that the distances of travel,
the brake application marks and the required vehicle stopping distances are clearly indicated
on a ramp for each vehicle to be tested. Cognicence must be taken of the fact that the test
vehicles are normally unloaded. (Not to exceed the length of the vehicles).

5. Any machine which is unable to stop within the laid down parameters will be rechecked,
repaired or have brakes adjusted prior to being allowed to proceed to work. (The vehicle
must stop within laid down parameters).

6. Vehicles used for the transport of explosives or persons will have braking systems checked
by the operator in the presence of the appointed Technician / Artisan prior to the
commencement of this task.

7. A signed copy of brake test results for each shift will be filed as directed by the Workshop /
Contracts Manager. The documentation will be retained for at least one month.

8. The Tramming Shift Supervisor and Team Supervisor will arrange the vehicle sequence for
brake testing as per planned areas to be loaded.

9. The assigned parking location for each vehicle must be such that brake testing sequence is
completed with the minimum delay.

10. Braking systems which must be tested include the foot as well as the park or emergency
brakes.

4.14.2 ADDITIONAL UNDERGROUND PRECAUTIONS

1. At each decline the operator will test both braking systems.

2. When moving from horizontal to inclined areas underground brakes must be tested.

3. At specified demarcated areas underground brake systems will be tested. (Frequency as per
Engineers directive and the location as per Engineer and / or Mine Overseers specifications).

4. Drill Rigs and Roof bolters will test braking systems against engine power prior to moving
out of park bays. At specified brake test sites holding power of brake system will be re -
tested prior to the machine proceeding to work site.

5. Any machine repaired underground due to a breakdown will where the effectiveness of the
braking system could have been affected re test brake holding power in the specified test site
as directed by the appointed Engineer.

4.14.3 GENERAL

1. The signed brake test sheet will be presented and analysed at any enquiry involving machines
i.e. Runaways, Collisions etc.

2. The operator will stop the machine should the brakes fail during the shift and park it where
it cannot run out of control.

3. Where no brake test sheet is made available to the Workshop / Contracts Manager or the
appointed Engineering Foreman / Technician for any particular shift the following must be
done :-

i. It must be established whether or not the machine was utilized during the shift in question.

ii. Who the operator of the machine was and which Supervisor was in charge of the shift.

iii. Why the check list was not completed by the operator and / or Artisan and / or Technician.

iv. The incident must be reported by Workshop / Contracts Manager or Engineering Foreman
together with the corrective action taken to prevent a re- occurrence at the morning meeting.

4.14.4 GENERAL (cont..)

4. No design changes or modifications to any of the braking systems of any vehicle will be made
unless these are approved by the appointed Engineer and Business Area Manager in writing.

5. It will be the responsibility of the person in charge of any recovery work to ensure that all
people and property are safe guarded before manually releasing the braking systems on any
vehicle.

6. No persons other than those authorised in writing to do so by the appointed Engineer or


Workshop / Contracts Manager may adjust or over ride any safety braking system on any
vehicle.
