QSP 5.4-01

• PURPOSE
• The objective of this QSP is to provide work
place standards to be followed by all FDA
personnel.
• This QSP also describes the procedures on
5S, Good Housekeeping Practice, safety and
security within FDA.
SCOPE
1. Shall apply to all FDA employees.
2. Define Workplace standards to be followed.
3. Conduct and Maintenance of 5 S
4. In coordination with AFO:
• Preventive maintenance and repairs of premises
including grounds, buildings/ facilities and vehicles
• Fire prevention and control
• Proper waste disposal and management
DEFINITION?
• Work Place Standard refers to the
criteria set on how each work place
should be or look like
• Work Place refers to the FDA premise
• Work Area refers to the immediate
area where an employee works
WHAT IS 5S?
SEIRI (Sort/ Suriin) refers to eliminating unnecessary items

SEITON (Systematize/ Sinupin) refers to arranging items in


good order

SEISO (Sweep/ Simutin) refers to cleaning the workplace

SEIKETSU (Standardize/ Siguraduhin ang Kalinisan) refers


to maintaining high standard of housekeeping and
workplace organization at all times.

SHITSUKE (Self-Discipline/ Sariling Kusa) refers to training


people to follow good housekeeping disciplines
independently and spontaneously without being told
WHAT ARE ITEMS?
Necessary Item refers to items,
supplies/materials, furniture, tools,
equipment and instrument directly used in
performing a job

Unnecessary Item refers to items not needed


in performing a job
UNNECESSARY ITEMS
Personal Items
• Picture Frames
• Large and excessive religious items
• Family pictures, clippings and mattings/ coverings under glass tops
• Company sponsored items (e.g. mugs, calendars, ballpens, fans, ID
lace, umbrellas, bags, planner, notebooks, memo pads)
• Slippers, shoes (not office prescribed), sandals
• Electrical kitchen and entertainment appliance (e.g. radio-cassette
player, component, electric fans, oven toaster, rice cooker,
microwave ovens, airpot)
• Trophies and medals
• Decorations like plastic plants, sculptures, figurines and the like
• Stuffed pillows, chair paddings and the like
• Sandals, slippers and excess shoes
• Merchandise
• Unlicensed firearms and deadly weapons
• Other colored ballpens except BLUE (Official), Liquid erasers
Individual work area
• Office desk / table and chair must be sturdy, durable and
stable.
• Working desk / table glass top (if present) must be clean
and be free of unnecessary materials (e.g. photos,
posters, clippings, mattings.
• Computers must be cleaned regularly specially the
keyboards and monitors.
• Pending documents, office supplies and other personal
items are placed inside the desk/table drawers well
organized, neatly arranged and labeled.
• No files/ documents/ records and unnecessary items
under the tables to provide ample leg room.
• Unnecessary personal items are not allowed (e.g.
slippers/ sandals/ excess shoes, figurines, picture frames,
stuff toys, seat cushions/ paddings).
• No eating and drinking allowed.
Comfort rooms & toilets
• Water supply is adequate
• Water dippers and pails provided in cases that flush
mechanism is out of order
• Floor is free from obstructions and water spillages.
• Door mat must be provided.
• Walls and exhaust fans must be clean and free of
cobwebs
• Ceilings are free from leaks
• Adequate and functional lights must be provided;
free from cobwebs, dust and dirt, any insects and
turned off when not in use
• Toilet bowl, urinals, and bidet must be clean,
odorless and functional. Flush mechanism / tank are
working.
• Stainless grab bar must be provided
• Sink and faucet must be clean and functional.
• Hand dryer must be functional.
• Liquid/ bar soap must be available.
• Trash cans are placed in the comfort rooms and its
cubicles; these must be emptied out at a regular
interval within the day.
• Mirrors must be clear and clean
• Doors and lock especially in cubicles must be
functioning
• Visible signages such as (1) proper hand washing
and toilet flushing, (2) Female and Male Washroom,
(3) conservation of water, (4) prohibition on
throwing tissues, panty shields and sanitary napkins
into toilet bowls, (5) out-of-order/ for maintenance,
etc.
• No Artificial plants and figurines inside the toilet
• Cleaning supplies and devices must be placed in a
separate area outside of the toilet.
• Provide with Maintenance Checklist on each room.
• Grab bar and signage for PWDs and Senior Citizens
inside a cubicle dedicated to them.
Records/storage
rooms(Central)
• No personal items, unofficial signages/ posters in
any side of the cabinet, shelves and racks
• Must be free from pest, insects, rodents, animal
droppings and termite traces
• Conduct regular pest control
• Lay-out (for Records and Sample storage rooms)
must be provided
• Records filing must be organized and properly
labeled
• Arrangement of supplies is where heavy objects are
placed in the lower shelves
• Air-conditioning unit functioning properly
• If no aircon, electric fans adequate in numbers,
clean and functioning
• Exhaust fans may also be installed
• Ceilings, walls and doors free from cobwebs, stains,
peels and markings
• Light diffuse, free from accumulated dust and dirt
• No busted and blinking lights
• Floors clean, no spillages or grease
Supply storage room
• No personal items, unofficial signages/ posters in any
side of the cabinet, shelves and racks
• Must be free from pest, insects, rodents, animal
droppings and termite traces
• Conduct regular pest control
• Office supplies must be organized and properly labeled
• Arrangement of supplies when stacked is where heavy
objects are placed in the bottom and light objects are
placed on top
• Air-conditioning unit functioning properly
• Ceilings, walls and doors free from cobwebs, stains,
peels and markings
• Light diffuse, free from accumulated dust and dirt
• No busted and blinking lights
• Floors clean, no spillages or grease
Passage ways
• Passageways are kept clear and free from any
obstruction
• Installed with fire extinguisher near the entrance /
door of the storage room
• Fire extinguisher must be regularly checked if
functional and not expired
Individual-dresscode
• Wear prescribed FDA uniform from Monday to Thursday
• Footwear:
• WOMEN- closed black leather/ synthetic shoes with at least one
inch heels
• MEN - black leather/ synthetic shoes except rubber shoes
• Employee ID must be worn at all times
• Friday attire:
• WOMEN - FDA Shirts with collar, blouse with sleeves, slacks,
denim pants, dress and skirt (hem maximum of 1 inch above the
knee)
• MEN - FDA shirts with collar, polo shirt with long or short sleeves,
denim pants, slacks
• No slippers, sandals, rubber shoes, plunging neckline,
backless, tight-fitting pants, hanging blouse, mini skirt,
walking shorts, leggings worn at any time within the
premises
Grooming & Hygeine
• Employee must have showered/ washed regularly,
no foul or objectionable odor specially when facing
clients
• Hair, moustache and beard must well-kept or
trimmed.
• Tattoos, piercings (other than ear piercing for
women) and other body decorations or art is not
allowed.
• For frontliners (those interacting with the clients),
must be presentable.
