common systems are described.
• Palmer notation system
• Universal numbering system
• Fdi system
• Numbering systems in dentistry serve as
abbreviations, instead of writing out the
entire name of a tooth, such as permanent
maxillary right central incisor, it is much
simpler to assign it a:
• Number
• Letter
• Symbol
Palmer Notation System
• Approved by ADA in 1947
• Tooth is represented by a number 1-8
(permanent) or a letter (A-E) (deciduous)
• Two lines indicates which quadrant the tooth
belong to
• A horizontal representing the occlusal plane
• A vertical represenating the midline
Universal Numbering System
• Palmer – Difficulty in keyboard typing
• ADA adopted the universal system in 1968
• Letters for deciduous teeth
• A-T
• Following a clock wise order from maxillary
right second molar to mandibular right second
molar
• Numbers for permanent teeth
• 1-32
• Following a clockwise order

• Proposed by FDI and Adopted by Who
• Each tooth is allocated a two – digit number
• Left designate the quadrant and the right
designates the tooth order
• Maxillary left deciduous lateral incisor 62
• Mandibular right permanent canine 43

(FDI).
• Some times we use the # before the two
digit(FDI) system to differentiate between
universal codes and fdi codes

• #22, permanent maxillary left lateral incisor

Introduction of Diagnostics Forms
• First you have to take Biodata of the patient.

• What you ask patient when taking Biodata

• Patient Name , Age , Gender , Class , School


2ND Step is Oral Examination
• In Oral Examination you have to examine all the
tooth in the oral cavity

• Start from the upper right quadrant than moves


toward the upper left quadrant than from lower
left quadrant towards the lower right quadrant.

• You Should examine all the soft tissue in the oral


cavity .
• In Tooth you have to examine whether they
are carious , retained milky tooth ,BDR .
• Whether there is any abnormality in teeth ,
plaque deposition is present or not , bleeding
gums present or not , stains
3Rd Step is Treatment Advised
• In this on your basis of oral Examination
Examiner advised the treatment to the patient

4TH Step is Oral Hygiene Report
• On the basis of examination examiner categorized
oral hygiene status of patient into
