• Assembling :The arrangement of medical records takes place in the
following order
• Coding : is the process of transforming descriptions of medical diagnoses
and procedures into universal medical code numbers.
(The main condition/principal diagnosis and procedure is coded by the MRO
or person given this responsibility. The diagnosis/procedure and code
numbers are entered into each individual patient’s admission record via a
computer terminal.)
• Filing : When completed, the medical record should be filed in data
