BASIC CLASSIFICATION OF IVF

• Normal Osmolality of plasma is 280 – 300 mOsm/L

(Isotonic/Iso osmotic)

• If less than 280 mOsm/L – hypotonic/hypo osmolar

• If more than 300 mOsm/L – hypertonic/hyper osmolar

VOLUME OF IVF
PARTS OF IV TUBINGS
COMMON PROBLEMS
1. IV stopped dripping
2. Air bubbles from IV tubing
3. Blood in IV tubing
4. Chamber drip too full
5. INFILTRATION/EXTRAVASATION
6. Over infusion or under infusion
IV COMPUTATION
• What is the volume of IVF?
• 50cc or ml
• 100cc or ml
• 250cc or ml
• 500cc or ml
• 1000cc or ml
• What type of infusion set is used?
• Macroset/macrodrip • 15 or 20 drops/cc
• Microset/ microdrip • 60 microdrops/cc
• Blood administration set • 10 or 20 drops/cc
• Soluset or VCAS • 60 microdrops/cc
cc –ml
Drops – gtts
Microdrops - µgtts or mcgtts
CALCULATION OF BASIC IV DRIPS
Volume of IV fluid X drop factor
IV drip = ___________________________________________________________________________________________

# of hours to be infuse X 60 min/hour

EXAMPLE: Start IVF D5NSS 1 liter to run for 10 hours using IV infusion set of 15gtts/ml
(macroset/macrodrip)
1000 ml X 15gtts/ml
= __________________________________________________________

10 hrs X 60 min/hr

= 15000gtts
________________________________

600min

= 25 gtts/min
CALCULATION OF BASIC IV DRIPS
Volume of IV fluid X drop factor
IV drip = ___________________________________________________________________________________________

# of hours to be infuse X 60 min/hour

EXAMPLE: Start IVF D5NSS 1 liter to run for 10 hours during IV infusion set of
60mgtts/ml (microset/microdrip)
1000 ml X 60mcgtts/ml
= ___________________________________________________________________________________________________________________________________________________________

10 hrs X 60 min/hr

= 1000mcgtts
___________________________________________________

10min

= 100 mcgtts/min
CALCULATION OF BASIC IV DRIPS
EXAMPLE: Start IVF D5NSS 1 liter to run for 10 hours during IV infusion set of
60mgtts/ml (microset/microdrip)

IV drip = Volume of IV fluid X drop factor

___________________________________________________________________________________________

# of hours to be infuse X 60 min/hour

10hrs

= 100ml/hr or 100cc/hr or 100mgtts/min

CALCULATION OF BASIC IV DRIPS
1. Start IVF of NSS 500 cc to run for 6 hours using
macroset with drop factor of 20gtts/ml. Compute for
IV rate
2. Shift IVF to D5NSS 250 cc to run for 8 hours using
microdrip. Compute for IV rate
3. Start Amoxicillin 500mg to be diluted in 100 ml of
NSS to run for 30mins, using soluset. Compute for IV
rate
4. Order: Infuse 1 unit packed RBC 250cc for 4 hours
using blood set with drop factor of 10gtts/ml
5. D5NM 1liter to run for 12 hours using macroset with
drop factor of 15gtts/ml
FIVE – PLUS FIVE RIGHTS OF
MEDICATION ADMINISTRATION
• The rights for medication • Additional rights for
administration are the professional nursing
foundation for practice
medication safety 1. Right assessment
1. Right patient 2. Right documentation
2. Right drug 3. Right to education
3. Right dose 4. Right evaluation
4. Right route 5. Patient’s right to refuse
5. Right time
ORAL MEDICATION
DRUG COMPUTATION
EXAMPLE:
• Paracetamol 750mg 1 tablet PO twice a day
• Stock Dose: 500mg/tablet
D

S
D – desired dose
S – stock dose/stock on hand
V – volume
Q - quantity
DRUG COMPUTATION
D
S X V or Q

EXAMPLE:

• Paracetamol 250mg/5ml syrup, give 3.7ml PO every 4

hours. Patient’s weight is 18.5 kg
• Recommended dose is 10mg/kg/dose

• How many mg/dose

• How mg/day
DRUG COMPUTATION
D
X V or Q
S
EXAMPLE:
• Give Paracetamol 10mg/kg/dose every for hours
for temperature 37.8˚C per orem. Patient’s wt is
32.4kg.

• Stock dose: 250mg/5ml syrup

• How many mg/dose
• How ml/dose
DRUG COMPUTATION

1. Order: Penicillin V 375 mg PO once a day

Stock dose: 250mg/tablet (scored)
a. How many tablet/s will you give?
