Podiatric (APPE) score:
A new diagnostic tool in suspected podiatric
acute appendicitis

INTRODUCTION

Acute appendicitis is acute inflammation and


infection of the vermiform appendicitis.
Appendix is a blind ending structure arising from
the caecum
Acute appendicitis is the most frequent
abdominal surgical emergency in the paediatric
age and it is diagnosed in 1 to 8 % of children
evaluated urgently for abdominal pain.
Base of developing an acute appendicular
inflammation is 7%
WORK UP OF PATIENT WITH ACUTE
APPENDICITIS

 Work up start with


1. clinical history
2. physical examination
3. laboratory test
clinical studies shown that imaging is a key strategy
in optimizing outcomes in appendicitis. Not only as an aid
in early diagnosis but also reducing negative
appendectomy rate.

APPENDITICIS PAEDIATRIC SCORE

 Aim in this investigation was to develop and validate a


new APPE score
 The score should help the clinician in managing patients
with abdominal pain suspected appendicular
inflammation.
MATERIAL AND METHOD

Setting
Setting was conducted at dep of paedtric
surgery Sants Orsola University hospital in Bolognas
Duration
January 2013 to Dec 2016
INCLUSION CRITERIA

Only patients operated upon


acute appendicitis were
included.
EXCLUSION CRITERIA

Excluded patients with previous appendectomy


and abdominal surgery
Chronic medical or malignant condition
Patients with recurrent abdominal pain and
patients with appendicular mass
STATISTIC ANALYSIS

Data was collected on excel worksheet using


SPSS software for comparison between two
groups AA and NA
T student test for quantitative variable
Chi square test and fisher’s test for categorical
variable were used
New score was obtained
LABORATOY TEST

 WBC Count ,CRP type of surgical approach stage of


inflammation on Histopathological examination
• catarrhal inflammation
• Phegmonous inflammation
• Gangrenous inflammation
• peritonitis
RESULTS

 Study population
 1462 clinical cases were identified
 Among them 437 were excluded because clinical data
was incomplete
 Final population was composed of 1025patients
classified into 2 groups
 829 patients with AA
 196 patients with NA
