Payment of Fees (pursuant to DAO 2004-16) * Application Fee - P 600.00 * Annual License /Permit Fee - P 900.00 (DRC below 24 cu.m.) - P 1,080.00 (DRC 24-47 cu.m.) - P 1,200.00 (DRC above 47 cu.m.) * Oath Fee - P 36.00 * Performance Bond (Cash) - P 1,000.00 * Performance Bond (P 720.00/cum. based on its DRC but not less than P 6,000.00 –if cash) * If surety bond (double the amount in cash) 3. Environmental Compliance Certificate (ECC); 4. Business Plan/Program duly prepared under the supervision of a Registered Forester; 5. Monthly Lumber/Logs Production Report; 6. Inventory Stock of lumbers/logs duly inspected/verified by the Inspecting Forest Officer to be attested by CENRO; 7. Mayor’s Permit/Business Permit (DTI for individual and SEC Registration for Corporation); 8. Tax Identification Number (TIN); and 9. Income Tax Return ADDITIONAL REQUIREMENTS (PER EO 23 AND MEMO DATED OCT.11, 2011 OF USEC FOR FIELD OPERATIONS: 1. Amended ECC to include condition on the installation of a web-based CCTV cameras within the WPP compound accessible to DENR; 2. Five Year Log Supply Contract; - 100% Timber Inventory Reports (Tally Sheets, Stand & Stock Table with electronic copy –CD/DVD ) 3. Map of the area with a scale of 1:10,000; 4. Subscribed and Sworn Field Verification/Validation Report as to the availability and sustainability of raw/wood materials; and 5.
