(RHU/CHO Staff, MSWDO/CSWDO, Academe, Religious Sector

Provide Support Services


Education (DepEd/ALS), Skills Training (TESDA),
Livelihood Programs (DOLE & DSWD),
Job Opportunities (Public & Private Sector)
CONT.

Moderate Risk

LGU shall provide


Outpatient Treatment & Rehabilitation Program
venue & other logistics
(2-3 sessions/week, minimum of 4 months)
for OP sessions

Stakeholders Conference for community reintegration


(at the barangay level)

Final Assessment & Issuance of Certificate of


Completion
(with the Composite Assessment Team)

Provide Support Services


Education (DepEd/ALS), Skills Training (TESDA),
Livelihood Programs (DOLE & DSWD),
Job Opportunities (Public & Private Sector)
CONT.
High Risk
LGU shall provide
Daytime Therapeutic Community Service house/building & other
(8 hrs/ day, 5 days/week, minimum of 6 months) logistics for the Daytime
TC Service and health
Stakeholders Conference for community reintegration workers who will run the
(at the barangay level) house

Aftercare Program
(2 sessions/week for 18 months)

Provide Support Services

Livelihood Programs (DOLE & DSWD),
Job Opportunities (Public & Private Sector)

Schisto Hospital for psychiatric management
or for management of medical comorbidities

RECOVERY SKILLS
- TOPIC DISCUSSIONS
- TWO (2) SESSIONS FAMILY GATHERING
AND UPDATES (2ND AND 4TH MONDAY)
EARLY RECOVERY SKILLS GROUP
SESSIONS
 Teaches clients essential set of skills for
establishing abstinence from drugs
 Itis a forum in which the counselor can work
closely with each client to assist the client
establish an initial recovery program

ND TH MONTH
- TWENTY-FOUR (24) SESSIONS OF
RELAPSE PREVENTION SKILLS
- TOPIC DISCUSSIONS
- SIX (6) MEETINGS OF FAMILY
EDUCATION GROUP SESSIONS
RELAPSE PREVENTION GROUP
SESSIONS
 Forumsin which clients may share
information about relapse prevention and
receive assistance in coping with the issues
of recovery and relapse prevention
FAMILY EDUCATION GROUP
SESSIONS
 Often the first group session attended by
client together with their families
 Providesa relatively nonthreatening
environment in which to present information
and provides an opportunity for clients and
their families to begin to feel comfortable
and welcome in the treatment facility
