• Douglas taken over by McDonnell in 1967
• McDonnell Douglas contracted convair for wide body jet 1968.
• Drafted FMEA and found 9 faults in 1969 but did not report of cargo door to FAA.
• Interchange of Memos between company for designing fault but nothing implemented.

• DC-10 faced accident over Windsor in 1972, no huge damage.

• FAA did not take the problem seriously and gave informal suggestions to change design.
• TAF 94 crashed over Paris 335 passenger died.
• in 1975 plane crashed and 275 died
 Cargo door open outward.
