Write the official name of the region where the rural health unit is
located. or shortcuts;

Stand-Alone means it has its own infrastructure, which is not
shared with any offices and services of the municipality / city.

Write the catchment population that this rural


health unit served.

• If there is only one rural health unit, write the entire


population of the municipality / city
• If there are two or more rural health unit in the municipality
/ city, get the population of each catchment barangay that
the rural health unit serve and sum the total.

existing sewerage system that will be brought to a sewage treatment plant.

SALARY PAID BY THE LGU / PG (NON-PERMANENT) – are HR that


have a non – permanent position and salary is paid by the LGU or
provincial government i.e JOB ORDER, CASUAL, CONTRACTUAL.

SCHEDULE IN THIS FACILITY ONLY (FULL TIME) – means that the


HR’s SCHEDULE is FULL TIME in this facility and doesn’t rotate or
have other schedules in other facilities.

SCHEDULE IN THIS FACILITY ONLY (PART TIME) – means that the


HR’s SCHEDULE is PART TIME in this facility and do rotate or have
other schedules in other facilities.

AUGMENTED BY THE DEPARTMENT OF HEALTH – are HR that are


being assigned to health facilities which salaries are paid up by
the Department of Health.

OTHERS – are HR that are being assigned to health facilities


which salaries are paid up by NGOs, faith based organization, etc.
• If there is a different operational hours write it down on OTHERS
(SPECIFY).
• To be filled up ONLY for rural health units with birthing home.
• Check NOT APPLICABLE if there is none.
