What is an INCIDENT?
INCIDENT:
An incident is referred to as a work-related event(s) in
which an injury or ill health (regardless of severity) or
fatality occurred, or could have occurred.
Incident can be categorized as:

1.ACCIDENT
2.NEAR MISS
What is an ACCIDENT?
ACCIDENT:
An "accident" is an unwanted
and sudden event resulting in
Property damage, physical
injury or fatality.

TYPES OF ACCIDENT:
•Death Disability
•Serious illness
•Injury
•Ill health due to exposure
•Asset Loss.
ACCIDENT
The two direct causes of an accident are:
1.Unsafe Acts
2.Unsafe Conditions

•Some research allocates the causes of accidents as


80 percent of accidents are due to unsafe acts and
only 20 percent to unsafe conditions.

•Corporate culture often plays a major role in the


occurrence of an accident.

•If experienced employees disregard the safety


regulations, new colleagues are inclined to do the
same.
UNSAFE ACT
An unsafe act is a conscious action by the
employee that creates new risks and therefore
the possibility of an accident.
UNSAFE CONDITION
An unsafe condition is a condition in which you
don´t have the risks under control and which
can therefore lead to an accident.
UNSAFE ACT &CONDITION
• Human failure can lead to unsafe actions
or situations.

• A lot of accidents are due to


carelessness.

• Check whether the machine you´re using


is safe and whether covers, guards and
other types of protection are still in place.

• A user manual should be provided with


every machine.

• All machines and tools must be well-


maintained and kept in perfect working
order.
ACCIDENTS PREVENTION
• A prevention policy has to be pro-
active.

• A company's management carries


considerable responsibility in
drawing up and implementing a
prevention policy.

• The direct managers have a major


part to play. The managers must
ensure that the existing rules and
regulations are observed.

• That involves carrying out regular


inspections.
REPORTING INCIDENTS
• Sometimes, attention is only
paid to industrial accidents in
which someone is (seriously)
injured.

• Accidents involving minor


injuries, however, occur ten
times more frequently than
serious accidents, but these
are generally not reported.
REPORTING INCIDENTS
• Provide a clear description of the
incident.

• Provide a clear description of the


location with date and time.

• Identify all items/material/ equipment


associated with the incident.

• Witness Detail.

• All employees IMMEDIATELY report


all incidents, including property
damage to the HSE department.

• Ensure employees receive training


on the incident
reporting/investigation procedure.
ACCIDENT TRIANGLE
• By investigating every minor accident and near-
accident without serious consequences, many unsafe
situations/actions can be highlighted.

• The accident triangle implies that many minor incidents


and less obvious risk situations ultimately lead to
serious accidents.
