HISTORY OF THE PATIENT

Dengue
• 47 year old man from endemic area
for dengue • Positive IgM
• Waiter in a local restaurant • Thrombocytopenia ( 67
× 109/L)
• Presented with,
- high grade fever(with rigors of 6
days) Leptospirosis
- throbbing headache
• Positive IgM dipstick
- periorbital pain test
- generalized body ache • Positive MAT On 8th and
- myalgia 25th day of illness
- fatigue
- anorexia
- No any bleeding tendencies or Malaria
skin rash
- No significant past medical • Positive blood smear for
history OR travel history. Plasmodium vivax
Differential Diagnosis

Dengue
Malaria Leptospirosis

Why it is not dengue ?

• Protracted febrile phase lasting beyond one week


• Absence of leucopenia and hemoconcentration
• Dengue IgM level can remain elevated for several months after exposure
• Dengue IgM test can cross react with nonflavivirus such as malaria, leptospirosis,
toxoplasmosis, syphilis.
