Acute sinusitis

Acute sinusitis
- Acute inflammation of sinus mucosa.
-most commonly involved sinus is maxillary (ethmoid> frontal>sphenoid sinus)
-Sinusitis maybe “open” or “closed”, depending whether the sinus cavity can
drain into the nasal cavity through the natural ostia.
Aetiology of sinusitis in general
• BACTERIOLOGY:
-Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis,
Streptococcus pyogenes, Satphylococcus aureus, Klebsiella pneumoniae
.Anaerobic infections are seen in sinusitis of dental origin.

A) EXCITING CAUSES :
-Nasal infections: Viral rhinitis followed by bacterial invasion.
- Swimming and diving: infected water enters sinuses through ostia.
- Trauma: Compound fractures or penetrating injuries.
- Dental infections. (maxillary sinus)
• B) PREDISPOSING CAUSES
LOCAL:
1- Obstruction to sinus ventilation and drainage
a)Deviated Nasal Septum,
b)hypertrophic turbinates
c) nasal polyp
d)edema of ostia due to allergy or vasomotor rhinitis
e) benign or malignant neoplasms).

2- Stasis of secretions in nasal cavity:


( Cystic fibrosis ,enlarged adenoids, choanal atresia)
3- Previous attacks of sinusitis

GENERAL
-Environment: Cold and wet climate.
- Poor general health: Exanthematous fever (measles,chickenpox),nutritional deficiencies, systemic disorders.
CLINICAL FEATURE:
symptoms:
-fever,malaise
-headache (frontal sinus- early morning headache, usually resolves in afternoon )
(ethmoidal sinus- pain betwen eyes ,headache while lying down)
(maxillary sinus-pain at temporal side)
( sphenoidal sinus- pain at vertex or occiput)
-nasal obstruction
-nasal discharge, usually purulent/mucopurulent

signs:
- tenderness at effected sinus
-pus in middle meatus by using anterior rhinoscopy
- post-nasal drip (purulent)
• DIAGNOSIS:
- Xray: WATER”S VIEW/LATERAL VIEW.
- CT is preferred.
-osteomeatal scan

• TREATMENT
-Antimicrobial drugs.
- Nasal decongestant drops.
- Steam inhalation.
- Analgesics.
- Hot fomentation.
Chronic sinusitis
1. symptoms:
-headache /dullness/heaviness of the head
-halitosis

2. signs:
-tenderness at effected sinus area
-mucoid like discharge seen in anterior rhinoscopy
- post-nasal drip seen

Management :
-besides medicine, functional endoscopic sinus
surgery is done
