• CSC Certificate;

• CSC map with proposed rehabilitation plan/development plan;

• Inventory report of the team (SUBSCRIBED and SWORN by the CENRO);

• Stand and Stock Table (SUBSCRIBED and SWORN by the CENRO);

• Tally Sheet (SUBSCRIBED and SWORN by the CENRO); Implement DAO 08 “Actual Diameter
(odd and even diameter in cms./Merchantable height at the first branch of the tree”.

• Electronic copy (CD) of inventoried trees to be harvested;

• Geo-tagged pictures of trees to be harvested and panoramic view of the area with coordinates;
and
• Environmental Compliance Certificate (ECC).
 FOR PEOPLES ORGANIZATION (CBFM)

1. PENRO endorsement;

4. People’s Organization application letter for harvesting;

5. CBFM Agreement;

6. Affirmation of Five Year Work Plan (FYWP);

7. The Five Year Work Plan;

• Development map (indicate the 1st up to 5th year of developmental activities in color
coding);

• Geo-tagged pictures of trees to be cut and panoramic view of the area with coordinates;

• Electronic copy (CD) of inventoried trees to be harvested;

8. Tally Sheet; (SUBSCRIBED and SWORN by the CENRO);
Implement DAO 08 (Actual Diameter: Odd and Even diameter
in cms./Merchantable height at the first branch of the tree).

Note:

• All documents coming from the requesting party should be stamped received by the
concerned CENRO (letter of intent, Certification from the Brgy. Captain, and other
related documents)

• All supporting documents in photo copies should be certified and authenticated as true
copies pursuant to Memorandum dated 05 August 2018 of FMB Director Nonito M.
