CHECKLIST OF REQUIREMENTS ON

THE REQUEST FOR TREE CUTTING

PERMIT FOR DPWH PROJECTS

1. Endorsement by DPWH Secretary per

Memorandum dated 11 August 2014 of the
DENR Secretary Ramon J.P. Paje;

2. Project Description/Development Plan;

3. Environmental Compliance Certificate

(ECC);
4. Map showing the relative location of the trees
affected by the project;

5. Letter consent and/or waiver from the

landowners affected, if there are any, together
with individual tally sheet of the trees found
thereat and copy of the land title;

6. Documentation/Comprehensive consultations
with all the concerned local governments units,
civil society organizations and
communities/stakeholders that there are no
outstanding nor anticipated issues against the
tree cutting; and
 
7. Favorable endorsements from the CENRO and
PENRO concerned, to include the basic Forestry
requirement such as;

• Inventory Reports to be signed by a

registered Forester
• Tally Sheets

• Stand and Stock Table

• Geo-tagged photos
