Does development of nursing disease management education influence hospital

readmission rates in patients with congestive heart failure?

Best Practice Implications
 Specific education program for nurses should be  Some studies had more men or more women, more of

readmissions from 25.4 % before the program to 9% African Americans than Hispanics, so more research
after program (Sterne et al, 2014) needs to be done on different populations.

decreased readmissions as well.

Conclusion:
Heart failure hospitalizations have the highest 30-day all causes readmission rate and the largest estimate total cost among
conditions in 2011 among Medicare beneficiaries (Murtaugh et al, 2017). With the numbers being so high, something needs to
change. Overall a number of factors could be changed, but starting with reevaluating the education programs for patients is a good
first step. More research will also have to be done on other populations to see how effective it really is.
