UNSAFE/ UNHEALTHY ACTS AND CONDITIONS

Prepared by:

What are unsafe acts and unsafe conditions?

Accidents are caused either by an unsafe act or an

unsafe condition or a combination of both. Accidents are
rarely caused by a dramatic failure of equipment and are
mainly caused due to human failure. Even an apparent
failure of equipment may be due to common factor in
failing to recognize signs of impending trouble.
ACCIDENT
An accident is an occurrence or circumstance
that is unintentional, unforeseen, unintended, and undesirable,
causing damage or loss of materials or property, injury, or death.

Types of Common Accidents:

• Dropping from height and falling (slips and

journeys) from the same level
• Struck against objects, structure, rigid, sharp, or
rough
• Hit by falling objects
• In, on or between stuff, captured
• Electrocution o
• The Fire
Costs of accidents
Injury costs are costs to be paid by businesses, whether directly or
indirectly. The cost of accidents can be best described by the
Iceberg Theory.

When an accident happens, cash must be spent on the

medical bills, insurance costs, and, in some situations,
penalty and legal expenses of the injured worker.
Companies also spend large amounts on restoring
damaged equipment and discarded raw materials. That
are what we consider to be the direct injury costs.
DIRECT COST
— costs that comes
directly from the
accident itself. Direct INDIRECT COST
cost of a workplace — costs which are not a direct
accidents include result of the accident. This is
compensation, outside medical
payments and medical compensation costs and this
costs (hospital stays is created by the accident
and ambulance fees) which is hidden or less
obvious.
INDIRECT COST INCLUDE:

1. Owing to job disruption on the day of the accident,

employees lose their productivity and wages. Lost or
poorer output of the wounded.

2. Lack of effectiveness due to work stoppage when

helping, among other workers, the injured worker,
inspection or simply out of interest. The accident's
psychological effect affects the productivity of the
workers.

3. Loss of efficiency among supervisors because they

spend their time helping the wounded, investigating the
accident, and writing inspection reports instead of
concentrating on managing employees and the workflow.

4. Hiring and educating substitute staff

5. Downtime due to malfunction of equipment.

UNSAFE/UNHEALTHY ACTS
This is defined as "any human action that breaches a
widely accepted safe work practice or standard
operating procedure" by the American National
Standards Institute (ANSI). This is an act by a worker
that does not comply with or deviates from an
established standard, rules, or policy. This often occurs
when there are incorrect attitudes, physical limitations
or lack of experience or data for a worker.

OTHER EXAMPLES OF
UNSAFE/UNHEALTH ACTS

• Non-wearing googgles and gloves

• Smoking near flammable or explosives
• Disregarding posted warning
• Wearing loose clothea while working on running mmachie
• Operating at unsafe speed
• Failure to wear a hard hat
• Operating equipment without authority
• Improper lifting
• Improper loading
• Failing to use PPE properly
UNSAFE/UNHEALTHY CONDITIONS
ANSI defines this as the physical or biological property of a substance,
machinery or atmosphere that is liable to cause injury to persons, harm
to property, disruption of operations in a facility or office, or other forms
of loss. Such cases can be covered or prevented.

OTHER EXAMPLES OF
UNSAFE/UNHEALTH CONDITIONS
• Unguarded rotating machinery
• Defective tools, equipments or materials
• Damaged electrical wires
• Oil or chemical spillage
• Machines produce excessive noise
• Inadequate or excessive lighting
• No safety training
• Faulty or expired fire extinguisher
• Improper storage of inflammable goods
• Unhygienic work environments
• Lack of emergency escape route
You should be as precise as possible in defining unsafe / unhealthy
acts and conditions. When referencing unsafe / unhealthy actions
and situations, you should stop using general terminology. Instead,
what you are referring to must be clearly mentioned, such as: dusty
office and improper paint storage.

Since these are the basis for management recommendations, it is

very necessary to state the unsafe / unhealthy actions and
conditions. Only a general suggestion or solution would mean a
general statement of the problem, not an accurate one. While this is
very simple, it is important to be transparent and precise in defining
unsafe / unhealthy actions and situations to reassure management
that their attention and dedication are worthy of safety and health
concerns in the workplace.
