Terjadi kecelakaan sekitar jam 20.30 wib di area sump. Seorang foreman Pit
Service bernama Andre Sanjaya bersama dengan driver sarana mengantar opt
dozer ke pondok disposal dalam kondisi masih hujan. Andre melihat tower
lamp yang berada di area sump bawah hampir terkena banjir. Lalu ia
mengoperasikan excavator yang berada di atasnya dan melakukan penarikan
tower lamp menjauhi sump. Selanjutnya, ia kembali menuju sump untuk
memeriksa pompa dan berenang tanpa menggunakan baju dan sepatu tetapi
hanya menggunakan celana pendek. Saat Andre hampir sampai di pompa, ia
berteriak meminta tolong kemudian driver sarana berenang untuk menolong
Andre, saat sebelum sampai Andre sudah tenggelam. Lalu driver sarana
mematikan pompa dan berteriak meminta tolong. Kemudian datang mekanik
dan beberapa unit sarana serta ambulans untuk melakukan pertolongan.
