An employee fell from a height of over 5 meters while working on an elevated road construction project after failing to properly attach his safety harness. Additionally, an employee working at a glove manufacturing plant suffered amputation of his right arm after his glove became caught in the rotating shaft of a washing brush tank while he was cleaning without first isolating the machinery. The investigation concluded a lack of lock out procedures and inadequate guarding of moving parts contributed to the accident. Finally, an employee walking to a nearby depot was fatally struck by a container truck. The trailer driver did not see the employee due to blind spots and a lack of separation between vehicle and pedestrian
