Lost Right Arm

What Happened:-
Employee 26-year Bangaldeshi,
working in gloves manufacturing plant
at glove production line. During
accident, removing debris of bypass
gloves at washing brush tank. He wear
glove, edge of glove stuck to the shat of
brush tank system, as shaft rotate, cause
right arm amputee.
Summary of Initial
Findings:-
 Fail to isolate system before
cleaning
 No adequate guarding to avoid
pinch points \ access to line of fire

Container Truck
What Happened:-
Employee was walking to the nearby
depot area \ store, as the employee
travel, a trailer moving towards the store
hit the employee. The employee sustain
major injury, later died at hospital.

Lesson Learn:-
 To keep eye during use the
pedestrian road and crossing road.
 Driver to “honk” at narrow road \
blind spot\ corners
 To assess the traffic and use
alternative pathway to get access to
