Các nhân nền
• Các nhân nền (basal nuclei) = thể vân
(corpus striatum) + nhân trước tường
(claustrum) + phức hợp hạnh nhân
(amydaloid complex)
• Thể vân được chia theo phôi thai & sinh lý
bệnh học thành 2 phần vân (striatum) và
nhạt (pallidum), chia theo cầu trúc thành
lưng (phần ở cao hơn) và bụng (phần ở
thấp hơn) -> tổng cộng gồm 4 phần
• Vân lưng (dorsal striatum) = nhân đuôi
(caudate nucleus) + bèo sẫm (putamen)
• Nhạt lưng (dorsal pallidum) = cầu nhạt
(globus pallidus), gồm cầu nhạt trong
(internal) và ngoài (external)
• Vân bụng (ventral striatum) = nhân nằm
(accumbens nucleus) + củ khứu giác
(olfactory tubercle)
• Nhạt bụng (ventral pallidum) = chất vô
danh (substantia inomminata)

Đặc điểm lâm sàng
• Trên 1 BN thường gặp phối hợp các động tác tự động
• Thường biểu hiện ở chi và mặt nhưng có thể ảnh hưởng
tới cơ hô hấp và cơ phát âm -> giọng nói khàn hoặc phát
âm tự động
• Thường biểu hiện lúc nghỉ, rõ hơn khi làm các nghiệm
pháp đánh lạc hướng, biến mất khi ngủ. BN không có khả
năng duy trì động tác
Cơ chế
• Tổn thương trực tiếp các cấu trúc: nhân đuôi, nhân bèo,
nhân dưới đồi thị, đồi thị và các đường liên hệ giữa chúng
• Tổn thương gián tiếp: mất ức chế tới phần nhạt (pallidum)
• Chất dẫn truyền: tăng hoạt động hệ dopamine
Nguyên nhân
• Biểu hiện giống nhau bất kể nguyên nhân -> định hướng
nguyên nhân theo đặc điểm khởi phát, tuổi, triệu chứng
kèm theo
• Nguyên phát (di truyền): gen thường trội hoặc lặn, liên kết
giới tính
• Tiến triển từ từ
• Đối xứng
• Thứ phát (mắc phải): bệnh mạch máu hệ thần kinh trung
ương, viêm/tự miễn, chuyển hóa và nội tiết, nhiễm khuẩn,
thuốc
• Tiến triển cấp hoặc bán cấp
• Không đối xứng hoặc chỉ biểu hiện ở 1 bên

Bệnh Huntington
• Tổn thương gen huntingtin ở đoạn ngắn NST số 4
• Tổn thương mất neuron ở nhân đuôi và bèo sẫm
• Lâm sàng: múa giật + suy giảm nhận thức + thay đổi
hành vi.
• Khởi phát: có thể gặp ở bất cứ lứa tuổi nào
• Người lớn: thường ở tuổi 30 – 50. Biểu hiện chính là múa giật
• Bệnh Huntington tuổi trẻ: khi bệnh khởi phát <20t, biểu hiện chủ
yếu là HC Parkinson + loạn trương lực cơ + giật cơ. Co giật cũng
thường gặp. Tiến triển nhanh
• Các HC giống bệnh Huntington: LS giống nhưng tổn
thương gen khác
Bệnh Wilson
• Múa giật là biểu hiện hiếm gặp của bệnh Wilson nhưng
không được bỏ qua do bệnh có thể điều trị được
• Nên đặt ra chẩn đoán khi BN <40t có động tác tự động
• Các triệu chứng thần kinh thường gặp của bệnh: run, thất
ngôn, thất điều, loạn trương lực cơ, HC Parkinson, chảy dãi
• Các triệu chứng thần kinh hiếm gặp: cứng hàm, giật cơ, múa
giật, giọng nói thều thào, co giật, thay đổi nhận thức – hành vi
• Vòng Kayser – Fleisher
• XN: giảm nồng độ ceruloplasmin máu (bình thường ở 5 –
15% BN), giảm nồng độ đồng huyết tương, tăng đồng niệu
24h, RL chức năng gan
• Chẩn đoán xác định: sinh thiết gan, xét nghiệm gen
Bệnh Hallervorden - Spatz
• Còn gọi là PKAN (pantothenate kinase – associated
neurodegeneration)
• Đột biến ở gen mã hóa pantothenate kinase 2 (PANK2)
gây tích tụ sắt ở nhân nền, chủ yếu ở cầu nhạt trong
• Khởi phát ở thập niên đầu tiên
• Biểu hiện lâm sàng: loạn trương lực dáng đi -> loạn
trương lực toàn thể, thất ngôn, HC Parkinson, thoái hóa
TB sắc tố võng mạc, suy giảm nhận thức, thay đổi hành vi
• MRI: dấu hiệu “mắt hổ”: tăng tín hiệu trên T2 vùng cầu
nhạt trong, bao quanh bởi một vòng giảm tín hiệu

Bệnh lý mạch máu
• Nhồi máu/ chảy máu vùng nhân nền hoặc thiếu máu vùng
này không biểu hiện trên lâm sàng
• Là nguyên nhân thường gặp nhất ở BN múa giật nửa người mắc
phải, gặp ở 1% bệnh nhân TBMMN
• Hiếm khi thấy tổn thương trên phim -> có khả năng tổn thường
đường dẫn truyền liên kết các vùng này
• Tiên lượng: phần lớn sẽ cải thiện sau 1-2 năm, có thể tồn
tại dai dẳng
Múa giật Sydenham
• Nguyên nhân thường gặp nhất ở trẻ em (chiếm tới 96% BN
múa giật cấp ở trẻ em)
• Do phản ứng chéo giữa kháng thể kháng liên cầu nhóm A với
kháng nguyên thể vân
• Thường gặp ở trẻ 5 – 15 tuổi, nữ: nam = 2:1
• Các biểu hiện chính thường xuất hiện sau nhiễm khuẩn 1 – 3
tuần: viêm đa khớp, viêm cơ tim, hồng ban vòng, hạt dưới da
• Múa giật thường xuất hiện sau 4 – 8 tuần, có thể tới 8 tháng,
diễn biến bán cấp, thường biểu hiện 2 bên (20-30% biểu hiện
nửa người)
• Yếu tố nguy cơ: viêm họng do liên cầu nhóm A tan máu beta,
thai nghén, dùng thuốc tránh thai
• Chẩn đoán theo tiêu chuẩn Jones được điều chỉnh năm 1982

Các hội chứng loạn TLC đơn độc
• Loạn TLC đơn độc toàn thể khởi phát sớm
• Khởi phát cục bộ ở tuổi nhi đồng/ thiếu niên, tiến triển đến toàn bộ,
có thể đơn lẻ hoặc có tính chất gia đình, di truyền hoặc không rõ
nguyên nhân
• Early-onset generalized dystonia (DYT-TOR1A)
• Adolescent-onset dystonia of mixed type (DYT-THAP1)
Các hội chứng loạn TLC đơn độc
• Loạn TLC đơn độc cục bộ hoặc phân đoạn khởi phát ở
tuổi trưởng thành
• Loạn TLC cục bộ hoặc phân đoạn, đơn độc, thường khởi phát sau
tuổi 30, thường đơn lẻ, hiếm khi tiến triển thành toàn thể nhưng có
thể lan đến các vùng cơ thể lân cận
• Adult-onset segmental dystonia (DYT-GNAL)
• Cervical dystonia
• Blepharospasm
• Writer's cramp
• Oromandibular dystonia
• Laryngeal dystonia (spasmodic dysphonia)
• Limb dystonia

• Loạn TLC – HC Parkinson: có thể kèm tổn thương hệ tháp và/hoặc các đặc điểm ngoài
vận động, bao gồm suy giảm nhận thức, phần lớn là di truyền
• Dopa-responsive dystonia (DYT-GCH1, DYT-TH, and DYT-SPR)
• Wilson disease
• Early-onset parkinsonism (PARK-PARKIN)

• Loạn động đột ngột kèm loạn TLC: đặc trưng bởi
những đợt loạn động tự phát hoặc do kích thích
kèm loạn TLC
• Paroxysmal nonkinesigenic dyskinesia (DYT-MR1)
• Paroxysmal kinesigenic choreoathetosis (DYT-PRRT2)
• Paroxysmal exertion-induced dyskinesia (DYT-SLC2A1)
Điều trị loạn TLC
• Là điều trị triệu chứng
