Action plan

1. Memberikan memo kepada dokter spesialis terkait informasi tentang presentase angka
rujukan ke RS luar yang cenderung meningkat, dan presentasi PRB yang belum
mencapai target (85%)
2. Menghimbau DPJP agar menuliskan secara lengkap indikasi dan diagnose pasien yang
akan dirujuk ke RS lain. Seperti: DM, jangan hanya tanpa komplikasi, karena dapat
ditangani di RSCC, tetapi dijelaskan komplikasi atau indikasi lain yang menyebabkan
pasien dirujuk ke RS lain
3. Pada kasus, pasien yang membawa SKDP dari RS lain, agar dilakukan pengecekkan
ulang, apakah masih harus dirujuk, atau sudah dapat ditangani di RSCC
Membuat grafik atau table rujukan per bulan mulai mei juni juli per dokter spesialis dan di
serahkan ke dokter yang bersangkutan dan diagnosis yang paling banyak dirujuk
berdasarkan dokter
Rapor ke dpjp 1 bulan sekali terkait angka rujukan
Harapan dari RS  menurunkan angka rujukan, yang sesuai dengan kompetensi dpjp dan
SD di RSCC
