    ToR of Regional/ District Consultant- FPCS-QIT

    Principle Responsibilities:
     Work under administrative control of DDFP
     Prepare advance tour program for next month on 25th day of the
    current month approved by the DDFP
     Send approved advance tour program to LD-CCSDP and Director
    (MCH-Services) & LD(MC-RAH)
     At least 12 or more facility visits per month. Minimum 1 Satellite
    Clinic, 1 Community Clinic, 4 UH&FWCs, 2 UHCs, MCWC,
    District Sadar Hospital and BAVS/NGO Clinics
     Generally 1 facility visit per day and maximum 2 facilities can be
    visited & include SSN as team member
     On arrival at the facility, brief the facility manager and other staff
    about the objectives of the visit
     Observe the overall condition of the facility, assess facility readiness in
    providing MCH-FP services, collect service data, observe counseling,
    client screening and service provision if available
