Principle Responsibilities-----Contd.
 Prepare an “ Action Plan” mentioning problem identified,
action taken locally, action needed from DGFP, responsible
person and time frame
 Keep a copy of the “ Action Plan” at the facility
 Compile “ Action Plans” of the conducted visits per month &
send to LD-CCSDP and Director (MCH-Services) on the 5th
day of next month with a forwarding letter
 Have discussion with the SSN and EMT about monthly visit
program findings & actions taken
 Guide EMT for maintenance of the equipments available in
different service delivery facilities
