Guideline for choosing how much blood to transfuse

.An example , to raise the hemoglobin level by 4g/dl in a patient weighing 40kg and
With a heamatocrit of 60% would require 560 ml .this calculation assumes a blood volume
of 70ml /kg. mean in this chart you just multiply the patient weight from your desire
increase hemoglobin , volume which shown in ml i.e. 14.0 x40=560ml ,
• if you increase 3g , Hematocrit 50% patient weight is 20kg so, 20x 12.6=252 for cross check
252/20=12.6ml/kg.
•If you increase 6 gram and hematocrit is 75% patient weight is 40kg check 1g increase at
75%=2.8ml/kg so 6x2.8=16.8 ml/kg then 16.8 x40=672ml is required volume of blood.

Calculation
Patient blood volume

= patient weight x surface area


=40kg x 70= 2800ml
Target increase hemoglobin (6grams)
= patient blood volume x target increase hb/ 100
=2800mlx6/100=168 grams
Donor Hb in blood unit
= volume of w/b drawn x Donor hb/100
=450 x14/100=63 %
Required blood volume
= pt target increase hb x packed cell volume/ packed cell hematocrit
= 168X 200/63 =533ml
Comparison
• From chart if patient weight is 40kg Target increase 3grams
unit hematocrit 75%volume required= 40 x 8.4= 336ml

• From calculation:
• Patient blood volume= 40x70=2800ml
• Target increase hb= 2800ml x3/100= 84
• Packed cell volume=300ml
• Donor hematocrit=75%
• so/, 84 x 300/75 =336ml
Its proven from both mehod the result will same
