COVID VACCINATION

PLAN
OBJECTIVE
To fully implement the vaccination program against covid virus in Lucena
City to a target population of 16 years old and above.

Specifically:
1. Provide masterlisting of target population
2. Install registration system
3. Conduct validation and screening process
4. Provide logistical requirements including venue preparation
5. Conduct pre and post information education campaigns

METHODOLOGY
The vaccination program shall be conducted in a fixed post identified as
____ public schools in Lucena City under the strict supervision of CHO
on a scheduled basis.

METHODOLOGY
Registration shall be made 14 days prior to the vaccination proper through the
BHERT and SPU. Priorities shall be given to the following:

1. Medical frontliners
2. Non-medical frontliners (government employees, uniformed personnel)
3. Other government employees including national offices
4. Senior citizens
5. Indigent families who acts the breadwinner of the family (Ycs, 4Ps)
6. Families of indigent breadwinners
7. Others that does not qualify from items 1-6
REGISTRATION
• For government employees: A masterlist of the target subjects shall
be submitted by the concerned offices/agencies. The list shall be
categorized as medical or non-medical employees providing frontline
service
• For the barangay residents: A masterlist shall be provided by the SPU
for yellow card holders and CSWDO for 4Ps beneficiaries
• For other sectors/uncategorized: the concerned barangays shall
submit the list to the CSWDO for validation
THE VACCINATION TEAM
• There shall be a total of _________teams for ___ sites during the

TASK GROUP LOGISTICS
• Preparation of Storage Facility:
• Target vaccines to be procured: 128,985
• Brand: Aztra Zeneca
• Recommended storage equipment: TWC 3000 medical refrigerators which can
store vaccines under 2 to 8 degrees Celsius
• For each site, there shall be a portable (??) storage equipment to store the
vaccines

• Preparation of the venue:
• Secure medical supplies (alcohol, syringes, cotton, PPE for moderate exposure, give-away kits
such as paracetamol, face mask, etc)

SUPPORT
• There shall be an standby ambulance for persons needing
psychosocial support or immediate medical care should a side effect
develops during the vaccination proper.
• The BLHP shall conduct hospital referral
• CSWDO together with DRRMO Emergency Rescue Team shall be
placed in strategic places to cover ____ sites.
PATIENT MANAGEMENT TASK GROUP
• Clients will be informed of his/her schedule and designated venue of
vaccination at least 2 weeks before the vaccination day.
• The clients were supposed to be validated and screened by the
BHERT/BHAs.
• Clients should bring with them their consent form. If client refused to
be vaccinated, he/she waived his privilege in the priority list should
he/she changed his/her mind
• The crowd control team shall maintain peace and order and ensure
that clients who come earlier than his/her schedule shall be advise to
return home and leave their number to be informed
PATIENT MANAGEMENT TASK GROUP
• There shall be a triage section for screening such as vital signs and
symptoms
• The information section explains through audio-video presentation of
the benefit and possible side effects of the vaccine, or the Q & A
portion
• After a client was vaccinated, there shall be a waiting area to observe
side effects. This shall last for 30 mins before he/she be advise to go
home.

• There shall be a SWD team to secure cleanliness of the venue
• Disinfection and Sanitation shall be conducted every 4 hours to
ensure safe environment
• All medical waste must be properly disposed of under CHO Sanitation
Unit supervision
• All non-medical waste must be disinfected prior to collection and
dumping at the landfill.
• No personnel shall be without mask and gloves
REPORTING AND MONITORING
• The TWG shall render post-activity report to the Local IATF within 15
days after the vaccination program with an assessment and
recommendations to aid future decisions.
