 SUBJECTIVE

 My subject is a 20 year old adult male, diagnosed with Schizotypal disorder and
Mixed obsessional thoughts and acts.
 Had no past medical or medication history.
 Suffers from psychotic symptoms for more than a year.
 Was prescribed with T.Olanzapine and T.Sertraline.
 OBJECTIVE
 To reduce Psychotic symptoms & to tailor the dose as per patient’s response.

Patient and care taker understands local language.
History about experiencing symptoms was
acknowledged through caretaker.
Diagnosis: Schizotypal disorder and Mixed
obsessional thoughts and acts.
No past medical or medication history.

T.Sertraline and T.Olanzapine are administered Might cause QT interval prolongation.

concurrently Monitor for ECG(initially after 1 week of use, later
after 1 month, followed by 3 months, 6 months and 1
year.)

Improvement in response to drug was noted

everyday.

Since Sertraline was well tolerated dose was

increased gradually.

 COUNSELLING
TO CARETAKER
 Monitor the patient closely as the drug might cause suicidal thoughts.
 Report symptoms like seizures, and burning sensation in thoracic region.
 May cause dry mouth,tremor,head ache,constipation,abdominal pain.
 Drug Olanzapine may cause weight,report when patient’s weight increases
abnormally.
 All the drugs on discharge should be taken at night after food with water.
 Do not miss any dose.
 Normal diet with sufficient amount of veggies and fruits to be followed.
