improved.

Outcome / Conclusion

 It is the nurse’s responsibility to communicate effectively, in a professional manner, to build


rapport and ensure that the family opens and feels comfortable explaining their habits
regardless if they are healthy or unhealthy.
 This family was able to express their concerns and daily routine to the nurse to develop an
effective plan of care and create solutions to the problem.
 The family was able to improve their sleeping patterns and improve their overall health
conditions.
 The nurse’s plan of care was effective and adequate for the family.
 The family was compliant with the plan of care and was able to see positive results.
