This phrase originated in the US film industry. The precept as it applies to films is as prevalent
now as it was in the silent film days. Many, in fact it is not too strong to say most, films aimed at a
young male audience involve plot devices that allow for action sequences which invariably end in
hot pursuits. There is usually a token love interest storyline before everything in sight ends up in
pieces.
The first reference to it dates back to that era, just after the first 'talkie' - The Jazz Singer, 1927. It
is a script direction from Joseph Patrick McEvoy's novel Hollywood Girl, 1929:
"Jannings escapes... _______________.“
There's quite a distance from a single citation in a script direction to a phrase that is part of the
language. It doesn't appear again in print for some years and we can be fairly sure that McEvoy
wasn't the source of the figurative use of the phrase as we now know it. The Winnipeg Free Press,
March 1944 ran an article about screen writing that included this:
Miss [Helen] Deutsch has another motto, which had to do with the writing of cinematic drama. It
also is on the wall where she cant miss seeing it, and it says: "When in doubt, ____________.“

The name X was often used as a
derogatory slang by American and Iraqi soldiers and their allies in the War in
Iraq to describe individuals suspected of a variety of offenses related to
theft and looting. British soldiers routinely used the term to refer to Iraqi
civilians. In the subsequent occupation it is used as a general term for the
insurgents, similar to Charlie for the Viet Cong in the Vietnam War.
Due to interaction of the two peoples, the term X was adopted by the Iraqis
to describe foreign troops suspected of looting, and the English-speaking
mainstream press mistakenly reported the slang to be native to the locals.
Iraqi citizens often use the term X to describe looting bandits, whether they
be coalition troops or insurgents of any nationality. For protection against X,
Iraqis are allowed one AK-47, and one 30 round magazine of ammunition
per household.

"It’s no good to write a deliberate anti-drug song and hope that it will
catch. Because the general thing is that people will be upset by that.
It would disturb them to have someone else shoving something
down their throat. So the best thing to do is offer something that
seems ambiguous—that on study or on reflection actually can be
seen to be ‘anti’—which the song X is actually an anti-drug song. If
you study it or look at it with a little bit of thought... from a
distance... or as it goes by... it just sounds like a song about X. But
actually, it is quite cleverly anti-X”
