COURSE OBJECTIVES

 To Recognize Where & Why Lockout/Tag out Is

Lockout
 Lockout is a technique used to prevent the release of
hazardous energy, or to prevent the hazardous energy
from escaping.
 A padlock is placed on the appropriate energy
isolating device that is in the off or closed position.
The “Fatal Five” Main Causes of Lockout/Tagout Injuries

 Failure to stop equipment


 Failure to disconnect from power source
 Failure to dissipate (bleed, neutralize) residual energy
 Accidental restarting of
equipment

before restarting
Definitions
 Authorized Employee - one who locks out machines or equipment in
order to perform the servicing or maintenance on that machine or
equipment.

Lockout Procedure For Tank Hydrotesting
 Alert the area supervisor that LOCK OUT/TAG
OUT Procedure will be conducted on the
concerned tank
 Clear the area of UNAUTHORIZED Personnel
 Blind all the shell nozzles and ensure that bolts
are properly tightened.
 Install the valve on the drain or filling line
 Conduct walkthrough inspection with the
Construction Superintendent, Quality Control
Inspector, and Hyrdrotest Supervisor. Ensure that
all those required to be blinded are done properly
and those that are not to be blinded are open and
there are no obstructions.
TAG OUT FORM
Lockout Procedure For Tank Hydrotesting
 IMPORTANT REMINDER: DO NOT BLIND ANY
ROOF NOZZLES DURING WATER FILLING
 Apply locks (Lock Out) using chains and padlocks at
the control point and on the tank filling/draining line.
There shall be four (4) locks to be applied:
 Hydrotest Supervisor
 Construction Superintendent
 Area HSE Officer
 Construction Manager
 Area HSE Officer to install TAG (Tag Out) and all key
holders to sign on the tag
Lockout Procedure For Tank Hydro testing
 Perform water filling and monitor continuously
(use logbook)
 Once water is full, close control valve
 Conduct pneumatic test as necessary
 Perform water draining as follows:
 Conduct walkdown and ensure that no vents are
blinded-off
 Open locks with concurrence from all lock holders:
Hydrotest Supervisor, Construction Superintendent,
Area HSE Officer, and Construction Manager)
 Sign on the TAG to concur removal of locks
 Conduct water draining
