1. RATIONALE
• Project TSEK (Track and Sweep Errors

Key) is a localized version of Error
Watch to expedite the response and
correction of errors from the
distributed SLMs aired TVBI, and RBI
episodes.

• Relative to OUA MEMO 00-1020-0138
defined as DepEd Error Watch Initiative
• The Division wants to ensure that errors
are tracked, controlled from being
repeated or distributed, and corrected.
• Aims to sustain the delivery of quality,
accessible, relevant and liberating basic
education services for all school-age
learners
• A team was created to cater error
reports.

2. PLEMENTATION MECHANISM

Error Watch Mechanism
1.Data gathering of errors
2.Use Enclosure 2 template filling out columns 1
and 2 and forward the submitted template to the
Focal Person.

3. The Focal Person will sort out the complaints and
forward the concern to the learning area in charge.
4. Subgroup head may or may not consult the LRMS to
verify assessment.
5. The subgroup shall then fill out column 3 of the
template and submit back to the Focal Person for
recording.

6. Focal Persons shall prepare missives informing the
concerned school or office of the error/s and the
recommendations needed. These reports shall also be
forwarded to the Monitoring and Evaluation section of
the SGOD for consolidation. That same section shall
submit weekly report or updates to the SDS.

In cases of reports that need to be
forwarded to the Central Office though their
Error Watch Initiative platforms, the focal
persons shall do the tasks.

Monitoring
• CID members are advised to monitor and
accomplish Enclosure 3 to check if concerned
schools or office use the recommendations to
check the error.
• Monitoring reports can be included in the weekly
monitoring report required from the CID
members.
