What is Questionnaire?
• A questionnaire is a paper-based or electronic tool for
collecting information about a particular research
interest.
• It is a list of questions or indicators that the participants
need to answer.
• In a quantitative study, a questionnaire typically uses a
scale (e.g. Likert Scale)
Types of Questionnaire
• Structured Questionnaire
• It uses closed-ended questions or indicators.
• Unstructured Questionnaire
• It allows participants to respond to open-ended questions.
• Semi-structured Questionnaire
• It have characteristics of both structured and unstructured types.

The proceeding slides contains the guidelines that you can
follow in using questionnaires for your data collection
procedure.
Guidelines in Using Questionnaires
• DECIDE ON THE METHOD OF ADMINISTERING THE
QUESTIONNAIRE
• Will you do it face-to-face or online? Use the face-to-face method if
you need to capture the non-verbal cues spontaneously displayed by
your participants as well as their emotions and behaviors. The online
method, on the other hand, involves the administering of
questionnaires through filling out web-based forms.
Guidelines in Using Questionnaires
• DRAFT YOUR QUESTIONNAIRE
• Divide your questionnaire into two or three parts:
• Personal Information Section – asks details about the participant’s
background which are relevant to the study.
• Main Section – lists the specific questions or indicators that are aligned
with the specific research questions.
• Open-ended Questions Section – asks additional information that may
not have been covered by the main section.

• DRAFT YOUR QUESTIONNAIRE
• Align the indicators or questions to your SOP or objectives.
• Provide clear instructions for answering the questionnaire.
• Use routing if there is a need to skip some items in the questionnaire.
• When several related questions need to be asked, start with the general
question first before the specific one.
• Do not make an over lengthy questionnaire.

• DRAFT YOUR QUESTIONNAIRE
• Make sure that the predetermined responses match the nature of the questions.
• Avoid using highly technical terms.
• Avoid using negative statements in your questionnaire.
• Avoid leading and biased questions
• Avoid double-barreled questions.
• Avoid overly sensitive questions.
• Use a reader-friendly layout.
Guidelines in Using Questionnaires
• PILOT-TEST YOUR QUESTIONNAIRE FIRST
• This is to ensure that the participants will understand
the questionnaire’s content. The pilot test can also
inform you if there are items or features of the
questionnaire which can be a source of inaccuracy in
the data.

• CONTACT THE PARTICIPANTS BEFORE
DISTRIBUTING THE QUESTIONNAIRES.
• Give them instructions to follow in answering the questionnaire.
• ATTACH A COVER LETTER TO THE
QUESTIONNAIRE WHEN CONDUCTING THE
ACTUAL DATA COLLECTION.
• It must contain the purpose, instructions, guarantee of confidentiality and procedure.

• FOLLOW UP THE PARTICIPANTS WHO FAIL TO
COMPLETE THEIR QUESTIONNAIRES BY THE SET
DEADLINE.
• IMMEDIATELY ENCODE THE DATA ONCE YOU HAVE
COLLECTED THEM AND ARCHIVE THEM
DIGITALLY.
