3. Grave Offenses which shall cause immediate termination


of contract or agreement:

- Gross neglect of duty


- Falsification of official document
- Receiving for personal use of a fee, gift or other valuable
things in the course of official duties, and committing acts
punishable under the anti-graft laws;
- Soliciting or accepting directly or indirectly, any gift,
gratuity, favor, entertainment, loan or anything of monetary
value in the course of duty
- Inefficiency and incompetence in the performance of duties
- Refusal to perform duty and assignments as provided in
the contract or agreement

4. Grave Offenses which shall cause immediate termination


of contract or agreement:

- Disclosing or misusing confidential or classified information


officially known to one’s office and not made available to
the public
- Commission of acts without prior notice and justifiable
grounds such as, but not limited to:

a. Incurring of at least (3) absences in a month, whether


consecutive or not;
b. Incurring of at least (3) tardiness in a month, whether
consecutive or not.

4. Other Conditions.

- The COSW shall not be entitled to any benefits due to


regular or casual employees
- The COSW shall be answerable for any claim for losses
and for damages to property of the office, its employees,
agents or third parties, or personal injury which may arise
from the own negligence or misbehavior of the COSW.
- If the COSW is to resign, or if his/her contract is
terminated/has arrived at the end date, he/she is to
properly turn over all materials, documents and information
under his/her position to the immediate supervisor. All
pending tasks assigned to him/her must first be completed.

• Behavioral Assessment of Contract of Service Worker
