TONSILLITES

Tonsillites
 Tonsillites is the inflammation of tonsils.
 Tonsils are two small masses of tissue at back side of throat.
Tonsils act as filters, trapping germs that can otherwise enter
into your airway and can cause infection. These also make
antibodies to fight against infection , as it is composed of
tissue similar to lymph nodes.
 Tonsillites are common especially in children.
Types of tonsiliitis.
 1. Acute tonsillitis .

 2. Recurrent tonsilitis.

 3. Chronic tonsillitis.

 4. Peritonsillar absess.
Etiology
 Important cause of tonsillitis is streptococcus . Most common is Group
A beta streptococci. It can be caused by viral illness like glandular fever
or micronucleosis or it can be caused by some other viruses like EBV ,
CMV , Adenovirus etc.
Pathophysiology
As bacteria or viruses enter into mouth or nose.

They get trapped in tonsils .

Tonsils work to destroy micro-organisms by wrapping them with


WBC’s.

Signs of inflammation like redness, swelling, fever and pain of tonsils.

TONSILLITIS
Clinical manifestations
 Swelling and pain in throat.
 Difficulty in swallowing (dysphagia).
 Reffered pain to ears.
 Fever and chills due to infection.
 Stiff neck.
 Blalitosis ( bad breath).
Diagnostic tests.
 History taking.
 Physical examination: we will do inspection, by inspecting inflamed
tonsils. We will do palpation to check increased size of tonsils.

 TESTS: we can diagnose the condition by performing throat culture, in


which we use cotton swab to take a sample from throat surface and
we will keep this sample in a cup to allow infectious agent to grow .
After that we will confirm , if it is bacterial infection or viral infection.
Treatment
 Treatment usually depends on cause and we will also provide
symptomatic treatment
 like for pain , we will provide analgesics (ibuprofen).
 If the cause is bacterial, then we will provide antibiotics like pencillin,
clarithromycin and erythromycin (in case pr. Is allergic to pencillin).
 If cause will be viral, body will fight off infection on its own.
Surgical treatment and nursing diagnosis.
 There is also surgical treatment that is tonsillectomy.
 NURSING DIAGNOSIS:
 Acute pain related to inflammation and surgical procedure.
 Impaired oral mucous membrane related to infectious process as
evidenced by inflamed tonsils.
 Risk of aspiration related to impaired swallowing.
 Deficit fluid volume related to inadequate oral intake secondary to
painful swallowing.
